A fatal accident inquiry is to be held into the death of a woman who took her own life and will examine the care provided by the health board.
Shannon Foy, 25, died at her home in Broxburn, West Lothian, on May 13 2021.
The Lord Advocate considers that her death occurred in circumstances giving rise to “serious public concern” and has decided that a discretionary fatal accident inquiry (FAI) should be held.
The Crown Office and Procurator Fiscal Service (COPFS) has lodged a first notice to begin the court process for the discretionary FAI.
The inquiry will explore the circumstances of Ms Foy’s death, the adequacy of the mental health care and treatment provided by NHS Lothian and whether opportunities to prevent her death were missed.
Andy Shanks, of the COPFS, Scotland’s death investigation authority, said: “The Lord Advocate considers that the tragic death of Shannon Foy occurred in circumstances giving rise to serious public concern and as such a discretionary fatal accident inquiry should be held.
“The lodging of the first notice enables FAI proceedings to commence under the direction of the sheriff.
“Shannon’s family will continue to be kept informed of significant developments as court proceedings progress.”
A preliminary hearing will take place at Livingston Sheriff Court on May 19.
Unlike criminal proceedings, FAIs are inquisitorial in nature, and are used to establish facts rather than to apportion blame.
NHS Lothian has been approached for comment.
