Gardaí are appealing for witnesses following two separate fatal road crashes in county Tipperary over the weekend.
Both incidents occurred in the early hours of Sunday, November 9, 2025, leaving a man in his 40s and a woman, Carmel Hackett, in her 80s, dead.
The first single-vehicle collision occurred in Toomevara, near Nenagh, sometime between midnight and 2:20am on Sunday. The sole occupant of that car, Carmel Hackett (née Dullea), of Coole, Toomevara, and formerly of Bandon, county Cork, was pronounced dead at the scene. Her body was removed to University Hospital Limerick for a post-mortem examination.
A second collision happened on the R445 at Birdhill. The car’s sole occupant, a man in his 40s, was pronounced dead at the scene. His body was taken to University Hospital Limerick, where a post-mortem examination will take place. The road has since reopened.

Mrs Hackett, who was predeceased by her husband Jimmy, is deeply mourned by her daughter Valerie, son William, grandchildren Hugh, Ellen, Tadhg, Jimmy, Kieran and Bridget, as well as her extended family and a wide circle of friends.
She is reposing at Ryan’s Funeral Home, Nenagh, on Tuesday evening from 5pm, with removal at 7pm to St Joseph’s Church, Toomevara, arriving at 7:30pm. Requiem Mass takes place on Wednesday at 11am, followed by burial in Toomevara churchyard.
Tributes have poured in online describing Carmel as “a real lady” and “full of fun and life”.
One friend wrote: “Many a great day and night spent in your home — she was such a character, full of fun and life. A very proud Cork woman.”
Another described her as “a great supporter of social dancing”, while a neighbour remembered “her warmth, humour and kindness to everyone she met”.
Others paid tribute to her generous spirit:
“She will be sadly missed by all who had the pleasure of knowing her. May her gentle soul rest in peace.”
The man killed in the Birdhill crash has not yet been named at the time of going to print. However, many in the area have also expressed sadness at the news of his passing, with one person writing: “May his soul rest in the arms of the angels. Condolences to his family.”
Gardaí are appealing to anyone who may have witnessed either collision, or who has dash-cam footage, to come forward.
In particular, investigators are asking road users who travelled on the R445 at Birdhill between 10pm on Saturday, November 8, and 8am on Sunday, November 9, to provide any relevant footage.
They are also appealing to anyone who was driving in the Toomevara area between midnight and 2:30am to contact them.
Tipperary North TD Ryan O’Meara offered his condolences, saying: “My sincere condolences go out to the families and communities bereaved over the weekend in the two separate road traffic accidents in Toomevara and Birdhill.
“I am personally aware of the impact that is being felt in Toomevara and Birdhill at such sad and sudden news. I know that the people of Toomevara and Birdhill will rally together as communities in the coming days as we come to terms with these heartbreaking losses.”
Local councillor Louise Morgan Walsh also commented: “Absolutely awful night on the roads around Nenagh.
“My sincere condolences to the families and friends of the deceased.”
